Symptoms of dry eye and vision-related quality of life in patients with keratoconus

Abstract Details

Purpose:

To evaluate symptoms of dry eye in patients with keratoconus (KC) compared with healthy patients. Secondary purpose of this study is to analize the vision related quality of life in patients with KC by using the National Eye Institute Visual Function Questionnaire-25 (NEI-VFQ-25).

Setting:

Methods:

A total of 120 patients with KC and 50 aged-matched healthy subjects were enrolled in this prospective study. All eyes were examined with a rotating Scheimplug camera (OCULOS - Pentacam) to assess KC severity. The subjective symptoms of dry eye were evaluated with a self-administred questionnaire of the Ocular Surface Disease Index (OSDI, Allergan, Inc Irvine, California). A subanalisis by stages (Amsler-Krumeich Classification) was done selecting the worst eye and excluding patients with ocular surgery. The Spanish version of the NEI-VFQ-25 was administred to evaluate potential association between KC disease and vision specific quality of life.

Results:

KC patients had significantly higher OSDI scores (3.42 25.05) compared with controls group (16.25 15.75) (p<0.01, independent sample t-test). Excluding eyes with surgical procedures: 38 patients were stage 1 (44.18%), 26 patients stage 2 (30.23%), 10 patients stage 3 (11.63%) and 12 patients stage 4 (13.95) by Amsler-Krumeich classification. OSDI score was worse in advanced stages: stage 1 (29.87 25,66), stage 2 (31.08 27.08), stage 3 (45.20 22,78) and stage 4 (43.22 19,13) (p<0,01, ANOVA test). The NEI-VFQ-25 composite score was 69.76 ± 19.49 in the KC group and 90.58 ± 7.08 in the control group (p <0.01, t-test).

Conclusions:

The OSDI questionnaire evaluated in this study indicate that KC patients suffer greater symptoms of dry eye compared with controls group. The analysis of NEI-VFQ-25 responses had revelated that vision related quality of life was worse in the KC group.
