Vitreoretinal (VIAs) and vitreomacular interface (VMI) abnormalities in eyes treated with endothelial keratoplasty

Purpose:

The purpose of this study is to evaluate the incidence of vitreoretinal interface and foveal pathology, such as: rhegmatogenous retinal detachment (RRD), vitreomacular traction (VMT), cystoid macular oedema (CMO), macular cysts, epiretinal membrane (ERM), full thickness macular hole (FTMH) formation (from traction on the posterior pole) and white-without pressure (WWP), lattice degeneration, atrophic round holes, retinal breaks (from traction at peripheral retina), vitreous haemorrhages (from traction at retinal vessels) following Endothelial Keratoplasty using spectral-domain optical coherence tomography (SD-OCT) and clinical records.

King's College Hospital, London,UK

Methods:

A retrospective, noncomparative, interventional case series involving 13 out of 361 patients who underwent endothelial keratoplasty at King's College Hospital between 2009 and 2016, and were subsequently referred to the VR department. Out of these 13 patients, 11 had undergone DSEK (Descement’s stripping Endothelial Keratoplasty), whilst 2 had undergone DMEK (Descement’s Membrane Endothelial Keratoplasty). Data were extracted from SD-OCT data ((Spectralis, Heidelberg Engineering, Heidelberg, Germany), electronic medical records (Medisoft) and paper notes from the ophthalmology department at King's College Hospital.

Results:

Out of the 13 patients referred to the VR department, 4 developed RRD, 3 developed vitreomacular traction, 2 developed vitreous haemorrhage, 1 developed CMO and 1 FTMH.

Conclusions:

Novel types of endothelial keratoplasty, combined or not with cataract surgery may be associated with various VIAs and foveal abnormalities. This case series emphasizes the importance of a thorough vitreoretinal examination in EK surgery candidates.
