Evaluation of examination and surgical treatment of children with glaucoma in East-Tallinn Central Hospital Eye Clinic

Abstract Details

Purpose:

To present the methods of examination and surgical treatment of children with glaucoma in East-Tallinn Central Hospital Eye Clinic.

Setting:

East-Tallinn Central Hospital Eye Clinic

Methods:

The most accurate measurements are obtained during examination under anesthesia. Visual acuity, pre- and postoperative IOP, mean corneal diameter and corneal status, axial length, cycloplegic refraction, gonioscopy were evaluated and imaging of optic disc with Ret-Cam was performed.

Results:

During the last 5 years we have seen 6 patients with different types of pediatric glaucoma: congenital glaucoma, congenital glaucoma in a premature child with ROP, secundary glaucoma due to uveitis, aphakic glaucoma and an interesting case of glaucoma due to acorea (absence of pupil). For diagnosis and treatment desicion we have performed examination under anesthesia to all the patients. Primarily the therapy is surgical: deep sclerectomy and trabeculotomy were performed on 6 patients. All the patients have good control of IOP during postoperative years. Only one patient needed topical treatment in a follow up period.

Conclusions:

For the patient with different types of pediatric glaucoma , the combination of deep sclerectomy with trabeculotomy is effective and provides reasonable control of IOP with few postoperative complications and need for additional medications. It is sufficiently predictable to consider such kind of surgical treatment in primary congenital or secondary glaucoma as the first choice.
