The study of ultrasound biometry effectiveness for optimal postoperative refraction in senile cataract surgery

Abstract Details
Purpose:
the comparative analysis of results of ultrasound and optical biometry in patients who underwent cataract surgery.
Setting:
The research was performed in the State Institution Eye Microsurgery Complex named after academician S.N. Fyodorov, Khabarovsk, Russia
Methods:
130 patients (130 eyes) with senile cataract were preoperatively examined by ultrasound (A/B scan “I-scan”) biometry. Optical biometry was performed in the same group of patients after phacoemulsification with IOL implantation.
The calculation of IOL optical power was done using Holladay-II program, the refractive target is emmetropia.
The values of axial length (AL) of the eyeball and postoperative refraction of pseudophakic eyes were also estimated.
Results:
The mean values of optical biometry made 20.9±2.4 mm and corresponded to the initial meanings of ultrasound examination with allowable error of 0.1 mm (equivalent to the refractive error of 0.5D) in 121 eyes (93.1%) with optimal and 2 eyes with myopic postoperative refraction (123 eyes – 96.4%). The same figures were higher on 0.5-0.8 mm compared to initial in 7 eyes with postoperative hyperopia. This can be explained by probable displacement of the sensor from the center of cornea and deviation of the examination axis from the central optical axis of the eye at preoperative measurement of AL.
Conclusions:
The performed analysis showed that preoperative ultrasound biometry in patients with senile cataract provided the optimal postoperative refraction in 93.1% of pseudophakic eyes.
Incorrect performing of ultrasound biometry before operation causes the induced postoperative hyperopia higher than 1D.
