Purpose:
To evaluate the results of intraocular lens (IOL) power calculation in eyes with radial keratotomy (RK) and dense cataract using ray tracing method in combination with immersion A-B – scan axial length (AL) measurements.

Methods:
This was a retrospective analysis of 20 patients (28 eyes with dense cataract) who had RK surgery in the '80s and '90s. IOL power calculation was performed using a ray tracing method in the Dual Scheimpflug and Placido system (Galilei G2, ZIEMER). AL was obtained from immersion A-B – scan method with prior localisation of the fovea (UD-6000, Tomey; Voluson 730 pro, GE Healthcare). The results of immersion A-B – biometry were compared with optical biometry (IOL-Master 500, ZEISS) postoperatively.
Results:
Dense lens opacities made it impossible to measure the axial eye length by optical biometry. Ultrasound immersion A-B – scan AL measurement with prior localization of the fovea was performed. The average value obtained by immersion A-B - method was 25.69 ± 0,17 mm. The mean value of control IOL-Master measurement was 25.71 ± 0,16 mm. Difference between postoperative optical biometry and ultrasound method we used was insignificant (P<0,001). We achieved target refraction within 0,5 D in 23 (82 %) cases, within 1,0 D in 28 (100 %) cases.
Conclusions:
Despite the high accuracy and reproducibility of optical biometry, an ultrasound method remains the only possible way for AL measurement in patients with dense cataract. Ray tracing IOL power calculation method in conjunction with ultrasound immersion A-B – scan AL measurement provides the precise result in patients with RK and dense cataract.
