Purpose:
To evaluate whether the effect of supplementary implantation of a Sulcoflex IOL (Rayner Intraocular Lenses Ltd.) in patients with negative dysphotopsia (ND) can be understood using individual biometry results or results of optical modelling.
Setting:
University Eye Clinic, Maastricht University Medical Centre, Maastricht, The Netherlands and Rayner Intraocular Lenses Ltd., East Sussex, United Kingdom.
Methods:
Ten eyes of eight patients with ND were treated with implantation of a Sulcoflex IOL. Biometric and perimetric measurements were used for evaluation of the treatment. Pre- and postoperative ray-tracing optical models of eyes with ND were constructed in the Zemax OpticStudio program using individual biometric data. Relationship between biometric parameters, ray-tracing data and ND course was evaluated using statistical analysis.
Results:
After surgery, ND resolved completely in six, partially in two and persisted in two eyes. Kinetic perimetry, performed in five eyes, corresponded to subjective ND course. There was no relationship between ND course and age, IOL power, or any of the biometry results taken individually. Ray tracing modelling has shown a decrease in light irradiance at the periphery relative to the centre of visual field. The peripheral light irradiance increased after surgery and the smallest increase was found in patients with persistent ND, compared to those with partial or complete ND resolution (p = 0.065 at 1.5 mm aperture).
Conclusions:
Whilst individual biometry results show no relationship with ND course, results of ray-tracing modelling, that combines all biometry results of a given patient, show encouraging correlation with ND course. This is opening the path to predicting the effect of supplementary IOL implantation on ND course using patient-specific optical models.
