Abstract Details

Purpose:

Accurate measurement of axial length (AL) is very important in achieving the desired refractive outcomes. The aim of this study was to evaluate the changes in AL measured with partial coherent interferometry (IOL Master 500, Carl Zeiss Meditec) and ocular biometric parameters with anterior segment optical coherence tomography (AS-OCT) device (Casia 2, Tomey) after cataract surgery in normal eyes.

Department of Ophthalmology, National Hospital Organization, Tokyo Medical Center, Japan

Methods:

In this retrospective study, 207 eyes of 159 patients who had undergone uncomplicated phacoemulsification and foldable intraocular lens implantation were enrolled. AL and keratometry were measured preoperatively and 1 month postoperatively. Central corneal thickness (CCT), anterior chamber depth (ACD: corneal endothelium to lens), and lens thickness (LT) were measured by AS-OCT, and intraocular pressure (IOP) was measured by non-contact tonometry. The relationships between the changes in AL and ocular biometric parameters were evaluated.

Results:

After cataract surgery, the mean AL was significantly reduced from 24.16 ± 1.71 (range 21.06 to 29.66) to 24.08 ± 1.70 mm (range 20.98 to 29.56) (p < .0001). The mean decrease in AL was 0.084 ± 0.047 mm. IOP was also significantly reduced from 14.8 to 12.9 mmHg (p < .0001), whereas ACD and CCT were increased from 2.64 to 4.12 mm (p < .0001) and from 535 to 547 μm (p < .0001), respectively. The change in AL significantly correlated with LT (r = 0.282, P < .0001), but did not correlated with the change in IOP.

Conclusions:

The difference in AL before and after cataract surgery might be related with measurement algorithm with a constant equivalent refractive index. The segmentation method with the adjusted refractive index for each part of the eye such as cornea, anterior chamber, lens, and vitreous might reduce the measurement error.
