Abstract Details

Purpose:

Maintaining good distance vision while improving the near vision is critical in patients who have presbyopia without the need for distance spectacles. Laser or Lens monovision is the conventional option for these patients. An alternative option is Monocular Refractive Lens Exchange using an ERV Lens. This paper reports on a series of patients with fully established presbyopia without significant defocus or astigmatism for distance who underwent monocular Refractive Lens Exchange using an ERV Lens.

Methods:

20 consecutive single surgeon operated patients who had fully established presbyopia but no significant refractive error for distance underwent monocular Refractive Lens Exchange in the non dominant eye only. The dominant eye was not operated on and remained phakic. All patients received an ERV Lens with a myopic target of less than 0.75 dioptres. 6-8 weeks after surgery they underwent a functional vision assessment using the PANFOCAL VA system. they also underwent an objective task assessment and were given a questionnaire asking them their need for spectacle use in different lighting conditions and for tasks at different distances.

Results:

Median and 95th percentile binocular UVA were 6/5 and 6/6 for distance and intermediate in photopic conditions, 6/7.5 and 6/9 for near in photopic conditions and 6/9 and 6/12 for near in mesopic conditions. Objective task assessment revealed that 95% or more patients successfully carried out intermediate and near vision tasks. All patients reported being spectacle free for distance and intermediate tasks in both photopic and mesopic conditions and near vision tasks in photopic conditions.

Conclusions:

Monocular Refractive Lens Exchange using an ERV Lens is effective and safe for presbyopic patients without significant defocus and astigmatism for distance vision. It provides for better distance vision compared to conventional monovision. All patients were satisfied with the outcome as indicated by objective and subjective measure of functional vision.
