Abstract Details

Purpose:

To evaluate the visual outcomes of myopic Small Incision Lenticule Extraction (SMILE) induced monovision in presbyopic patients.

Setting:

1) University Eye Hospital, Ludwig-Maximilians University, Munich, Germany 2) SMILE Eyes Clinic Linz, Austria

Methods:

A total of 48 consecutive presbyopic patients older than 45 years seeking surgical correction of myopia or myopic astigmatism were subjected to bilateral SMILE with planned monovision. Target refraction was plano for dominant (distance) eyes and ranged between -1.25 and -0.50 diopters (D) for non-dominant (near) eyes. Best-corrected distance visual acuity (BCVA), uncorrected distance visual acuity (UDVA), uncorrected near visual acuity (UNVA) as well as spectacle dependence were collected after a mean postoperative period of 7.2 ± 4.8 months. In addition, induced corneal higher order aberrations were evaluated using a Scheimpflug camera (Pentacam HR; Oculus Optikgeräte, Wetzlar, Germany).

Results:

Mean age was 49±3 years and mean spherical equivalent of surgical refractive correction was -4.70±1.92 diopters. Preliminary results show that 95.8% of distance eyes achieved a spherical equivalent correction of ±0.50D from target refraction and 97.9% were within ±1.0D. Binocular UDVA of 20/20 or better was achieved by 87.2% of patients and all patients achieved 20/25 or better. Binocularly, 78.7% of patients achieved UNVA of J1, 91.5% could read J2 and all patients could read J3 or better. The proportion of patients with binocular UCVA of 20/20 or better and UNVA of J3 or better amounted to 95.8%.

Conclusions:

Our data demonstrates that SMILE monovision represents a well-tolerated and effective approach for the correction of presbyopia in patients with myopia and myopic astigmatism considering refractive surgery. To our knowledge, this is the first study to investigate the viability of SMILE monovision.
