Evaluation of glistenings in two different types of intraocular lens using Scheimpflug technique

Abstract Details

Purpose:

To compare the development of glistenigs in two different hydrophobic acrylic intraocular lens (IOLs) by a quantitative and a semiquantitative examination mode.

Setting:

Péterfy Sándor Street Hospital-Clinic and Accident-Emergency Centre, Budapest, Hungary

Methods:

IOLs were implanted during standard phacoemulsification cataract surgery 4 years previously. Inclusion criteria: uncomplicated cataract surgery, that process the anterior capsulorhexis covers 360° in the edge of the IOL ’s optic. Exclusion criteria: corneal or retinal pathology, glaucoma, other ocular surgery than previous cataract surgery, YAG-capsulotomy. The examination included measurement of the best-corrected visual acuity (BCVA) and contrast sensitivity. In the semiquantitative mode, glistenings were graded subjectively at the slitlamp biomicroscope. In the quantitative mode, the Scheimpflug images of the IOL performed by Pentacam HR device ,were imported for objectively quantifying using the ImageJ digital image analysis computer software.

Results:

25 eyes of 25 patients (Alcon SN60WF IOL) and 26 eyes of 26 patients (Medicontur 860FAB IOL) were examined. By the semiquantitave mode statistically significantly more glistenings (p<0,0001) were observed in the SN60WF IOLs (1,82(± 0,9)) than in the 860FAB IOLs (0,58(± 0,6)). By the quantitave mode statistically significantly more glistenings (p<0,0001) were observed at the SN60WF IOLs (39,7(± 3,7)) than at the 860FAB IOLs (34,1(± 1,6)). Regarding BCVA and contrast sensitivity the differences between the two types of IOLs were not statistically significantly. There was a positive correlation between the two types of diagnostic methods (r= 0,446).

Conclusions:

Statistically significantly different glistenings values were observed in the two different types of hydrophobic IOLs by both the slitlamp biomicroscope grading and the Scheimpflug image analysis. More glistenigs were observed in the SN60WF IOLs by both quantification methods. The both diagnostic methods showed the same tendency. The Scheimpflug images analysis quantification method is a new, innovative technique with wich objective and quantitaive measurement datas can be obtained. This method could be adopted either for everyday use or for scientific measurements
