Comparison of two multifocal IOL types: long-term visual outcomes

Abstract Details

Purpose:

To compare the refractive and visual outcomes collected during the first postoperative year, after the implantation of two different types of one-piece acrylic diffractive multifocal intraocular lenses (IOL).

Setting:

Semmelweis University, Department of Ophthalmology, Budapest, Hungary

Methods:

Phacoemulsification was performed on 100 eyes of 50 patients. Medicontur Bi-Flex677MY IOL was implanted in 50 eyes of 25 patients (Group A), and Alcon Acrysof RestorSN6AD1 IOL, into 50 eyes of 25 other patients (Group B). Optical biometry was used for IOL planning and for the follow-up of biometric parameters. Postoperative examinations were performed 6 and 12 months after the surgery. Clinical outcomes were collected including uncorrected and corrected visual acuity for 4 meters, 80 and 40 centimeters, furthermore the measured and manifest refraction. Depth of focus was characterized using defocus curve. Patients’ satisfaction was tested with visual functioning questionnaire.

Results:

Uncorrected and corrected visual acuity significantly increased in both study groups, for all three analysed distances (eg.1 year postop. unilateral UDVA (A): 0.00±0.04; (B): 0.07±0.11; UIVA (A): 0.26±0.13; (B): 0.33±0.12; UNVA (A): 0.16±0.06; (B): 0.23±0.10). Manifest refraction showed significant reduction in both cohorts (Wilcoxon; p<0.05). Uncorrected VA was found significantly better in Group A, for all three distances (Mann-Whitney). Significant (34.0%) difference was found between the area under the defocus curves (Group A: 5.71 D; Group B: 4.26 D; Mann-Whitney, p<0.001). Evaluation of VFQs showed similarly good outcomes in both groups.

Conclusions:

Multifocal IOL implantation is a well-calculable and lastingly efficient method for the cataract surgery of patients who wish to achieve spectacle independence. In Group A, superior uncorrected vision and better depth of focus was found 12 months after surgery, compared to patients in Group B.
