Clinical performance under dim illumination conditions of a FineVision® trifocal IOL after clear lens extraction: 1 month pilot study

Abstract Details

Purpose:

The purpose of this study was to evaluate short-term changes in visual function and dysphotopsias under low illumination conditions in refractive lens exchange (RLE) presbyopic patients implanted with a trifocal IOLs.

Setting:

Hospital Privado da Boa Nova and Hospital da Luz, Portugal.

Methods:

Sixteen eyes of 8 patients candidates to presbyopic RLE were bilaterally implanted with a Trifocal IOLs (FineVision, Physiol, Belgium). Distance VA was measured under photopic conditions and contrast sensitivity was measured without and with intensive glare in dim illumination conditions (Functional Visual Analyzer, StereoOptical, IL). Light disturbances were evaluated under monocular and binocular conditions with a prototype device (Light Distortion Analyzer, University of Minho, Portugal). Subjective quality of vision was also assessed with the Quality of Vision (QoV) questionnaire obtaining scores of Frequency, Severity and Bothersome. All procedures were conducted before surgery and 1 month after implantation.

Results:

Defocus curves at 1 month revealed that no clinical or statistically significant difference was present in any vergence and shows the stability of the best correction at distance. CSF under photopic and mesopic with glare conditions shows a statistically significant improvement for the frequency of 3.00 cycles per degree (p=<0.05, Wilcoxon Test). Light distortion under monocular conditions showed a significant increase from baseline to 1 month visit and this change was statistically significant (p<0.05, Wilcoxon). In every visit, it was observed a binocular summation effect that reduced the LDI parameter and more significant at 1 month visit.

Conclusions:

Trifocal IOLs RLE is an effective technique in clear lens patients for presbyopic correction to achieve spectacle independence however, significant light distortion issues may subjectively reported specially after the first month after surgery. Longer follow-up is required to assess for potential improvements that justify the longer term satisfaction reported by most of such patients in clinical practice.
