Evaluation of corneal power measurements obtained by three different devices

Abstract Details

Purpose:

This study aims to compare different measurements of healthy anterior segment structures with three devices and determine whether these instruments can be used interchangeably in the clinical practice. For this purpose, corneal measurements (corneal power, and astigmatism) from Galilei, IOL Master and the topographer Modi02 (CSO, Scandicci, Italy) were compared. Additionally, anterior chamber depth (ACD) values obtained with Galilei and IOL master were compared.

Setting:

The study took place in a private clinic - Center for Excellence in Ophthalmology, located at Pará de Minas, Brazil.

Methods:

Seventy three eyes of 42 patients with cataract, which underwent an ophthalmic examination in the period of October 2014 to May 2015 were evaluated. The criteria for inclusion in this study were: healthy eyes, despite the cataracts, without any prior eye surgery. Patients with incomplete records were excluded from the analysis. Descriptive statistics for all variables was done. Subsequently, the measurements obtained by the three devices were compared in pairs using non-parametric statistics (Wilcoxon paired with a confidence interval estimated by Hodges-Lehman method), Spearman correlation coefficient and Bland-Altman plots. Significance level of 0.05 for the interpretation was adopted.

Results:

The highest value was from Medi02 (44.00±1.58) and the lowest was from IOLMaster (43.49±1.52). About Steep SimK, the highest value obtained was from Medi02 (44.82±1.59) and de lowest from Galilei (44.40±1.62). Corneal astigmatism indicates similar values of Galilei and Medi02 (0.84D and 0.82D, respectively), both lower than IOL Master (1.04±0.72). Statisticaly significant difference was observed between the corneal astigmatism obtained by IOL Master and Medi02 and when comparing Galilei and IOLMaster. ACD in IOL Master (2.95±0.32) was lower than in Galilei (3.08±0.30) with statistically significant difference and strong correlation and good agreement.

Conclusions:

The study demonstrated clinicaly small differences in astigmatism between the three devices. The same was observed in ACD when comparing Galilei and IOL Master. Good correlation and agreement was observed too, what make the devices interchangeably in pratical clinic.
