Abstract Details

Purpose:

To report the 5-year outcomes of ultrathin Descemet Stripping Automated Endothelial Keratoplasty (UT-DSAEK) utilizing grafts with a central thickness intended to be within 100 µm

Setting:

Villa Igea Private Hospital (Forlì, Italy)

Methods:

The medical charts of all eyes that underwent UT-DSAEK consecutively and had a minimum 5-year follow-up were reviewed. Main outcomes measures were best spectacle-corrected visual acuity (BSCVA), endothelial cell density (ECD), as well as graft survival rate and rejection cumulative probability rates at 1,3 and 5 years.

Results:

Central graft thickness within 100 µm was found in 275 of 295 eyes. After exclusion of co-morbidities, 1,3 and 5 years post-UT DSAEK outcomes included: BSCVA ≥20/20 in 36.3%, 46.4% and 53.4% of eyes respectively and ≥20/40 in in 95.5%, 92.0% and 96.6% of eyes; mean ECD was 35.4%, 42.3% and 52.3% respectively; survival probability was 99.1%, 94.2%, and 94.2 % respectively; and immunologic rejection probability was 3.4%, 5% and 6.9% respectively.

Conclusions:

UT-DSAEK grafts allow excellent long-term outcomes, including BSCVA, ECD and survival rates comparable to those recorded after DMEK with slightly higher immunologic rejection rates.
