Purpose:
To compare the visual outcomes of Verion Assisted Arcuate Incisions, Verion Assisted Manual Arcuarte & Toric IOLs for management of coexisting cataract and corneal astigmatism, ranging from 0.75 D to 1.50 D
Setting:
Comparative case series of 144 eyes which underwent Cataract Surgery with Astigmatic correction by Verion Guided FLACS (Group-1)(48 eyes) and Verion Guided Manual Arcuate (Group-2)(48 eyes) with Toric IOL (Group-3) (48 eyes). Implantation period is January - October 2016 at a Tertiary Care Eye Hospital, Ranchi, India
Methods:
Retrospective review assessed outcomes of Verion Assisted FLACS Arcuate Incisions, Verion Assisted Manual Arcuate Incisions with aspheric Monofocal Toric IOLs for visually significant cataract (NS Grade 3) and coexisting corneal astigmatism of – 0.75 D to -1.50 D under single operating surgeon.
Planning of Arcuate Incisions, planning and implantation of Toric IOLs were done with reference of Verion Image System.
Visual Recording and Aberrometry (I Trace) were done at 7th and 90th days post-operatively.
The main outcome measures were 7 days and 90 days post-operative UCVA and Contrast Sensitivity in Photopic and Mesopic conditions expressed in Modular Transfer Function.
Results:
At day-7, ANOVA analysis for CS at 5CPD and at 10 CPD for photopic and mesopic conditions shows no difference among the three groups.
However Group-3 showed significantly better results than Group-1 & Group-2 at day-90.
At 5CPD, Mean difference between Group-3 vs Group-1 for photopic (0.0277,p=0.007,95%CI(0.0076,0.0478)) and mesopic condition (0.0292,p= 0.001,95%CI( 0.0117,0.0466)) & between Group-3 vs Group-2 for photopic (0.0279,p=0.007,95%CI(0.0078,0.048)) and mesopic condition (0.0321,p= 0.000,95%CI( 0.0146,0.0495)) are significant.
Similarly at 10CPD, Mean difference between group-3 vs Group-1 for photopic (0.03542,p=0.000,95%CI(0.0181,0.0527)) and mesopic condition (0.03104,p=0.001,95%CI( 0.0138,0.0483)) & between Group-3 vs Group-2 for photopic (0.03583,p=0.000,95%CI(0.0186,0.0531)) and mesopic condition (00.03146,p=0.000,95%CI(0.0142,0.0487)) are significant.
Conclusions:
Toric IOL implantation and peripheral corneal relaxing incisions yielded similar results regarding surgical correction of astigmatism at the time of phacoemulsification cataract surgery.
All the three surgical modalities for correction of astigmatism achieved comparable results at 7 days post op but the astigmatism correction was less effective in due course of time in the Flacs Arcuate incison group and Manual Arcuate Incision group as compared to the Toric IOLs.
