Abstract Details

Purpose:

To investigate refractive outcomes and induction of corneal higher order aberrations (HOA) in eyes, that underwent single step TransPRK Xtra for Myopia / Astigmatism on borderline corneas not candidate for LASIK using smart pulse technology and an aberration neutral profile centred at the corneal vertex with asymmetric offset.

Setting:

Muscat Eye Laser Center, Muscat, Sultanate of Oman.

Methods:

Retrospective consecutive chart review of 74 patients (104 eyes) with Myopia/Astigmatism on borderline corneas that underwent TransPRK Xtra performed by two surgeons using the AMARIS 1050RS excimer laser platform (SCHWIND eye-tech-solutions, Kleinostheim, Germany) combined with simultaneous high energy accelerated corneal crosslinking using the KXL device (Avedro), were retrospectively analysed. Only eyes targeted for plano were included in the analysis. The preoperative metrics were correlated with the outcomes at 12-months postoperatively.

Results:

The mean spherical equivalent was -4.10±1.31 D preoperatively, and -0.07±0.14D at 12-month postoperative visit (p<0.0001). Mean refractive astigmatism was 1.15±1.14 D preoperatively, and 0.11±0.35 D at 12-month postoperative visit. Postoperative UDVA of 20/25 or better was achieved in 97% of eyes, at 12-month, compared to 98% CDVA preoperatively at 20/25 or better. Statistically significant correlation was observed between preoperative and postoperative aberration values for vertical trefoil (p<0.0005), vertical coma (p<0.0005), oblique tetrafoil (p<0.0001), secondary cardinal astigmatism (p<0.05), cardinal tetrafoil (p<0.05), and secondary vertical trefoil (p<0.05).

Conclusions:

Single step TransPRK Xtra for Myopia / Astigmatism on borderline corneas not candidate for LASIK, using corneal vertex centration with asymmetric offset is safe and predictable. Centering on corneal vertex may reduce the induction of coma compared to non-aspheric profiles
