Abstract Details
Purpose:
To compare the visual and refractive outcomes of iris-claw phakic intraocular lens (pIOL) implantation and laser-assisted subepithelial keratectomy (LASEK) , for correction of high myopia.
Setting:
Hanbit-eye clinic , Jeonju, South Korea
Methods:
The present study included 60 eyes of 30 patients who underwent pIOL implantation in 1 eye and LASEK was performed in the contralateral eye. We retrospectively examined preoperative and postoperative uncorrected visual acuity (UCVA), best corrected visual acuity (BCVA), manifest refraction spherical equivalent (MRSE), corneal endothelial cell density (ECD), the percentage of corneal endothelial cell loss, and changes in anterior chamber depth. High order aberration (HOA) was also compared. Outcome measures were evaluated at 1, 3, 6, 12, and 24 months after surgery.
Results:
There were no statistical differences in UCVA, BCVA, MRSE, ACD, or endothelial cell loss in either
the pIOL implantation and LASEK(all p >.05). The preoperative mean endothelial cell density in pIOL implantation was 3303±230 cells/mm2 and decreased 1.2% on the final follow-up(3299±261cells/mm2). No significant endothelial cell loss was observed after pIOL implantation or LASEK at any time of evaluation (P>.05). Induced amount of spherical aberration (SA) was significantly lower in the pIOL implantation than LASEK (P<.05).
Conclusions:
Based on 2-years results, visual and refractive outcomes were comparable with pIOL implantation and LASEK .Iris –claw pIOL implantation did not produce significant corneal endothelial cell loss after surgery compared with LASEK. Although pIOL implantation and LASEK gave excellent results, pIOL implantation provided quicker visual recovery, lower induction of spherical aberration, and achieved better predictability and refractive stability than LASEK.
