Comparison of corneal endothelium in uncontrolled diabetes mellitus and normal population

Abstract Details
Purpose:
Diabetes mellitus (DM) effects the eye as well as the other organ systems. Previous studies demonstrated DM may change corneal parameters. Our aim is to investigate the changes in corneal endothelial parameters in patients with uncontrolled type 2 DM.
Setting:
Gaziantep University, Department of Ophthalmology, Gaziantep, Turkey.
Methods:
Patients who had DM for at least 10 years and HbA1c level more than 9% were included in this study. Healthy participants formed the control group. Exclusion criteria were previous intraocular surgery, intraocular injection, laser photocoagulation, contact lenses or any other ocular pathology. All participants underwent complete ophthalmologic examination including best corrected visual aquity (BCVA), intraocular pressure (IOP), slit-lamp biomicroscopy, fundus examination and central and paracentral corneal endothelial evaluation using Nidek CEM-530 specular microscope. Right eye of each patient were included in the study unless it had an exclusion criteria. Corneal endothelial parameters were compared between the groups.
Results:
Thirty one uncontrolled DM patients and 30 volunteers were enrolled in this study. There wasn’t any difference regarding age, gender, IOP between groups. BCVA was significantly lower in DM group. Mean onset of DM was 11,27±2,97 years, mean HbA1c level was 11,02±1,49% in the DM group. Mean central cell density (CD) was 2513,6±260,75 in DM group and 2602,1±245,84 in control group. There wasn’t any statistically significant difference between the groups regarding the central and paracentral corneal endothelial parameters like number of the cells, CD, coefficient of variation and hexagonality.
Conclusions:
There wasn’t any significant difference in corneal endothelial parameters between two groups. Being an avascular tissue may help to protect the cornea from the devastating effects of DM until the later stages of the disease. Further studies are needed to fully understand the relation between DM and corneal endothelium.
