Abstract Details

Purpose:

To describe the incidence of a fluid retention syndrome with related increase in intraocular pressure in association with Small Incision Lenticule Extraction.

Setting:

Clearview Institute

Methods:

It was a retrospective and observational case series. All patients underwent preoperative and postoperative refractions, topographies, complete ocular examinations and anterior segment OCT's.

Results:

5 patients (3 males and 2 females) were identified, who experienced a significant IOP increase post-Small Incision Lenticule extraction (SMILE) with an associated myopic shift. Their mean refractive correction was -7.35 SE and they all were originally misdiagnosed as having diffuse lamellar keratitis. There was a myopic shift ranging from -1 to -4 diopters and a central IOP of 22-24 mm Hg with a peripheral IOP of 28-32 mm Hg. They were all treated topical Pilocarpine 1% and combined Timolol/Brominidine. Their uncorrected visual acuity returned to 20/20 or better with a final refraction from -.25 to +.25.

Conclusions:

A fluid retention syndrome post SMILE can be a rare complication. It is likely a steroid response with an underestimation of the IOP centrally. It appears to resolve without long term consequences once identified and treated appropriately and promptly.
