Purpose:
Purpose: To compare the clinical changes occurring after small incision lenticule extraction (SMILE) and wavefront-guided (WFG) laser in situ keratomileusis (LASIK) in eyes with myopic astigmatism.
Setting:
SMILEEYES Augen+Laserzentrum Leipzig Germany
Methods:
Prospective consecutive comparative study including 38 eyes undergoing WFG LASIK with the STAR S4IR excimer laser (Johnson and Johnson Vision, Santa Ana, Calif) (WFG LASIK group) and 52 eyes undergoing SMILE with the Visumax platform (Carl Zeiss Meditec, Jena, Germany) (SMILE group). Visual, refractive, ocular aberrometric (iDesign, Johnson and Johnson Vision), corneal topographic and pachymetric (Pentacam, Oculus), and dry eye symptomatology (Ocular Surface Disease Index, OSDI) outcomes were evaluated during a 6-month follow-up. A self-developed questionnaire was used to evaluate postoperative satisfaction and subjective visual quality.
Results:
Significantly better uncorrected distance visual acuity (UDVA) LogMAR was found in WFG LASIK group vs SMILE group at 1 day, 0.04 (0.09) compared to 0.13 (0.09), and 1 week, -0.01 (0.06) compared to 0.05 (0.07) postoperatively (p<0.001). At 1 day after surgery, significantly higher levels of sphere (p=0.018) and cylinder (p=0.044) were found in SMILE group. Significantly higher levels of primary spherical aberration and comawere found in SMILE group during the whole follow-up (p≤0.014). No significant postoperative differences were found in OSDI (p≥0.239) and subjective questionnaire outcomes (p≥0.086).
Conclusions:
Conclusions: SMILE and WFG LASIK are efficacious and safe procedures for the correction of myopic astigmatism, providing similar levels of satisfaction and dry eye symptomatology. WFG LASIK provides an initial better visual outcome and a lower induction of High Order Aberrations.
