Effective capsulorhexis size and postoperative changes with laser-assisted capsulotomy

Abstract Details

Purpose:

To determine the effective capsulorhexis size and its postoperative changes with femtosecond laser assisted cataract surgery (FLACS).

Setting:

Clearview Institute

Methods:

Patients were prospectively enrolled and followed. A capsulotomy was programmed for 4.8 mm. At the end of the case the video images were analyzed using the IOL optic size as a reference point and slit lamp pictures taken at month 1,3 and 12 for postoperative analysis. Complete ocular examinations, ocular wavefront, topography were obtained preoperatively and at month 1, 3 and 12. Preoperative and intraoperative lens thickness measures, anterior segment OCT and axial length measures were obtained.

Results:

A total of 64 patients were enrolled. 59/64 completed the 12 months follow up. The end of the case capsulotomy size ranged from 4.85 to 5.25 mm with a mean of 5.05 mm ( p<.01). At month 3 the range was 4.7 to 4.95 mm with a mean of 4.85 mm (p<.01). At month 12 it ranged from 4.5 to 4.85 mm with a mean of 4.7 mm (P<.01). A younger age (p<.03), greater lens thickness (p<.01) and greater axial length (p<.01) were associated with a larger capsulotomy.

Conclusions:

There appeared to be a variation from predicted with the initial capsulotomy size with FLACS. It returned to planned by month 3 though contracted further by month 12. In this study sample it did not have any negative clinical consequences, but it would suggest to plan for a capsulotomy size of less than 5.4 mm. It remains to be seen whether or not the trend for further contraction will stabilize after month 12.
