Comparison of corneal tunnel incisions created with femtosecond laser and manual keratome according to optical coherence tomography

Abstract Details

Purpose:

To estimate morphological structure and configuration of corneal incisions created with femtosecond laser in the early postoperative period compared to manual incisions with standard disposable metal keratome according to anterior segment OCT.

Setting:

IRTC Eye Microsurgery Ekaterinburg Center

Methods:

The study included 53 eyes of 52 patients, mean age 64 ± 10.4 years. The patients underwent phaco through 2.2-mm incisions. In the study group (23 eyes of 22 patients) corneal tunnel incisions were created with LensX femtosecond laser (Alcon, USA), in the control group (30 eyes of 30 patients) corneal tunnel incisions were created manually with standard disposable metal keratome by one experienced surgeon. Two-plane incision was planned in both groups. To compare configuration and morphological structure of the incisions, OCT images of the anterior segment were taken at first day post-op (RTVue-100, Optovue, USA).

Results:

Mean incision length in the study group was 1545 ± 110 microns. In all cases the incisions had two-plane profile. Local Descemet’s membrane detachment was found in 7 cases (30,4%), internal diastasis in 18 cases (78,3%). In the control group, mean incision length was 1524 ± 225 microns. Two-plane profile of the incision was found in 3 eyes (10%), in the rest of eyes the incision had arcuate one-plane profile. Local Descemet’s membrane detachment was found in 21 case (70%), internal diastasis in 21 case (70%). Local Descemet’s membrane detachment occurred less frequently in the study group (p<0.05).

Conclusions:

Femtosecond laser gives a possibility to create corneal tunnel incisions of a given configuration with stable length parameter preset in the treatment plan according to intraoperative OCT. Corneal tunnel incisions created with femtosecond laser preserve given configuration after phaco and IOL implantation. Local Descemet’s membrane detachment was less frequent un incisions created with femtosecond laser compared to metal keratome.
