Outcome of cataract surgery for unilateral congenital or developmental cataract

Methods:
A retrospective charts review of 70 consecutive children that underwent unilateral cataract surgery was performed for visually significant congenital or developmental cataract in the first four year of life. Group A: comprised of infants presenting up to one year of age, Group B: More than twelve to 24 months of age, and Group C: children with age group more than 24 months to 4 years of age.The primary outcome measures were refractive error changes and final VA achieved at last follow-up visit. The secondary outcome measures were visual axis clarity and evaluation of complications.

Results
Fifty-nine children met inclusion criteria. Mean age at presentation was 14.27 months (range 1 month - < 4 years) and mean follow- up period was 22.58 months (range 12-60 months). Sixty one percent of children presented during their first year of life, 20.3% in second year and 18.6% in third year of life. Of 17 children who co-operated for visual acuity evaluations, 52.94% achieved best corrected visual acuity of 20/80 or better. Twenty one patients (35.59%) required a total of 27 additional secondary intraocular procedures of which 15 were surgical capsulotomies for visual axis obscuration.
Conclusions:
The visually significant unilateral congenital/developmental cataract should be promptly dealt with surgery followed by occlusion therapy to improve visual acuity and to prevent amblyopia. There is a strong positive correlation between early cataract surgery for both monocular and bilateral congenital cataract and a better visual outcome
