Abstract Details
Purpose:
To analyze the outcome of explantation of multifocal lens implants used consecutively in a high volume chain of refractive clinics. Special focus on explantation rate, indications, complications and outcomes in terms of BCVA and patient satisfaction. Estimate of costs related to repeat operations.
Setting:
The Danish branch of the Memira clinics (private refractive surgery).
Methods:
Review of 139 multifocal lens explantations in 100 refractive patients treated in our clinics from January 2009 through December 2016. Analysis of relief of dissatisfying symptoms. Comparison of explantation rate for each make of implant.
Results:
A total of 7948 multifocal lens implantations were performed. The overall explantation rate in our clinics was 1,75 % with variations among implants. Indications for explantation comprised haloes and glare, poor visual quality, unsatisfactory reading ability, low contrast sensitivity and refractive surprise. For secondary implant, a monofocal lens was selected in the majority of cases (90%). Complications were few, nine eyes lost 2 lines or more. Success rate: 73% of patients expressed high or moderate satisfaction with the outcome. However, in 13 eyes no relief of symptoms was reported . Chair time was unusually long. Compared to the normal follow-up plan, these patients consumed between 8 and 65 extra visits.
Conclusions:
In spite of surgical success and relief of primary symptoms, some disappointed RLE customers may remain dissatisfied after explantation of the multifocal lens . Surgical safety is adequate, but timing of secondary intervention is crucial. A trend toward to shorter observation time before explantation was seen over the study period, leading to a higher degree of patient satisfaction. Caring for the dissatisfied multifocal RLE customer is time consuming.
