Refractive corneal inlay implantation to improve near vision in emmetropic presbyopic patients

Abstract Details

Purpose:

To report the clinical outcomes and safety of refractive corneal inlay (Presbia Flexivue MicroLens) for presbyopia

Setting:

1. Department of Ophthalmology, Samsung Medical Center, Sungkyunkwan University School of Medicine 2. Busan BalGeun Se Sang Eye Clinic 3. Seoul BalGeun Se Sang Eye Clinic

Methods:

Thirty one emmetropic presbyopes (range: -1 to +1D) were implanted with the in the non-dominant eyes as a prospective study. Monocular and binocular visual acuities at near and far distance and refractive errors were evaluated. Corneal astigmatism and anterior and posterior corneal curvatures were obtained from Schiempflug imaging. Contrast sensitivity, anterior segment optical coherence tomography were performed preoperatively and at postoperative 6months. And corneal nerves were analyzed with confocal microscopy, and dry eye evaluation was done by measuring tear break-up time, Schirmer test and corneal staining score.

Results:

At 6 months, uncorrected near visual acuity was improved from 0.52 ± 0.13 preoperatively to 0.26 ± 0.08 in operated eyes (P<0.01). Mean binocular uncorrected distance visual acuity was not altered (P=0.716) whereas uncorrected and corrected distance visual acuity in operated eyes were significantly reduced from 0.01 ± 0.03 and -0.01 ± 0.02 to 0.35 ± 0.20 and 0.17 ± 0.13, respectively (P<0.01). Tear break-up time, Schirmer test, and staining score got worse until postoperative 1month, However after postoperative 3 and 6 month they got better and resembled preoperative. Corneal nerve density measured with confocal microscopy was slightly decreased (p=0.06).

Conclusions:

The refractive corneal inlay is a safe, simple, and efficient method for correcting near visual acuity in patients who had presbyopia
