Purpose:
To evaluate long term visual and morphological outcomes as well as patient satisfaction and spectacle independence after PAD Mf-IOL implantation in patients with cataract, operated with and without PCCC.
Setting:
Csolnoky Ferenc University Teaching Hospital, Veszprem
Methods:
Fifty cataract patients with presumably healthy fundi had bilateral phacoemulsification and PAD-Mf-IOL (BiFlex-M) implantation. For each patient one eye was operated on with PCCC whereas the fellow eye without PCCC. During the 24 months follow up we tested the uncorrected and corrected visual acuities for distance (UDVA/CDVA), intermediate (UIVA/CIVA), and near (UNVA/CNVA); residual refractive errors; mesopic defocus curve; and contrast sensitivities for mesopic, photopic and backlight conditions. Patient satisfaction was evaluated with VFQ-25 test, with particular regard to visual artefacts. Position of the lens was tested and photographed with undilated and dilated pupil.
Results:
Functional results of patients that developed pathologies independent from operation were not evaluated. Median visual acuities for no PCCC/ PCCC were UDVA: 0.00/0.00, UIVA: 0.1249/0.0629, and UNVA: 0.069/0.069. Residual refractive error under ±0,50 diopter was achieved over 90% for all distances. Contrast sensitivity levels were high. Backlight artefact symptoms were reported mild/moderate/serious in 34/6%/2% respectively. Spectacle independence for distance and near was achieved in 100%, and 97% for intermediaer vision. IOL-s rotated 3.61 ±2.34 degrees between postoperation day and 1 months, 0 degree thereafter. Patient satisfaction rate was 95%. There was no clinically significant difference among PCCC and no-PCCC eyes.
Conclusions:
2 year results of 98 PAD-Mf-IOL (BiFlex-M) implantation provided nearly total glasses indepence for distance, intermediate and near vision in this study. Visual artefacts were of low level and easy to tolerate, improving with time. The position of the lens in the bag was stable. PCCC did not have significant influence to visual functions and lens position. YAG-laser capsulotomy was performed in 9 cases of no-PCCC eyes at 2 year follow up. Suboptimal functions were measured only in patients with minor non cataract-related (mostly macular) alterations discovered during follow up.
