Abstract Details

Purpose:

To describe the efficacy of Combined subscleral trabeculectomy and Express Shunt in Refractory glaucoma .

Setting:

Keywords: intraocular pressure – Express- Trabeculectomy-Refractory

Methods:

This prospective study included 30 eyes in 30 patients with complicated uncontrolled glaucoma with intraocular pressure (IOP) more than 20 mm Hg despite maximal topical antiglaucoma medications. All patients subjected to Combined subscleral trabeculectomy and Express Shunt in Refractory glaucoma. The preoperative measures included preoperative IOP, postoperative Snellen best corrected visual acuity (VA), slit lamp examination. The postoperative measures included postoperative IOP on day 1, 1 week, 1 month, and every 3 months till one year, postoperative Snellen best corrected visual acuity, slit lamp examination were recorded and postoperative complications were noticed.

Results:

This study revealed a highly significant reduction in IOP in first postoperative day, after one month , after 3 months and after one year (p value = 0.001 ) with maximum reduction seen in group E (pseudoexfoliation glaucoma) as postoperative IOP was 30.33+4.63 that was declined to 14.23+1.72 after one year ( p value = 0.0001) while minimum reduction reported in group D ( pseudophakic glaucoma) where IOP preoperatively was 28.67+6.80 that was decreased to 15.83+1.94 ( p value = 0.004) .Low incidence of postoperative complications including hyphema(2 cases),increased IOP ( 6 cases) and hypotony ( 1 case).

Conclusions:

Combined subscleral trabeculectomy and Express Shunt achieved a statistically significant reduction in IOP in refractory glaucoma with limited postoperative complications.
