PreserFlo Microshunt - the better trabeculectomy? First experiences with a new microshunt in surgical glaucoma therapy

Abstract Details

Purpose:

The PreserFlo Microshunt (from Santen) is a new system for lowering the intraocular pressure via a subtenonal drain. In contrast to the known methods of minimally invasive glaucoma surgery, the implantation takes place externally. We present our 1-year results on the effectiveness of pressure reduction, safety and complications as well as post-operative treatment in patients with POWG- glaucoma and PEX glaucoma.

Methods:

We have implanted the Preserflo Microshunt in 130 eyes of 96 patients. We monitored the corresponding patients with regard to intraocular pressure, visual acuity and medication consumption. Regular monitoring of the filter zone by swept-source-OCT was also performed, with a known tendency to fibrosis or onset of fibrosis, subconjunctival MMC administration.

Results:

All eyes showed a significant reduction in pressure during the postoperative observation period. The mean initial pressure while maintaining the antiglaucomatous medication was 28.3 mmHg. Postoperatively, the intraocular pressure was 9.6 mmHg on average after 12 month. The number of medium-sized drugs decreased from 2.66 to 0.06, a reduction of 99%. After a reduction, the visual acuity recovered to the original visual acuity within 2-3 weeks. Needling was required in 46 eyes (35.4%) and in 50% of the cases this was combined with 5-FU injection and in 15 eyes (6.9%) an open revision of the fibrosed filter zone was required.

Conclusions:

After one year, the PreserFlo MicroShunt shows a very effective and lasting pressure reduction. On the other hand, the complication and revision rate in our hands is significantly lower than with comparable interventions. After our results of one year postoperatively, we can say that the PreserFlo Microshunt - is the better trabeculectomy
