Prevention of intraoperative myosis during FLACS

Abstract Details

Purpose:

Development of a method of intraoperative myosis prevention during femtosecond laser- assisted cataract (FLACS) surgery without the additional use of medications and mechanical devices

Setting:

Konovalov Eye Clinic, Moscow Russia

Methods:

The prospective open-label study included 98 patients (98 eyes) with presbyopia who underwent femtosecond laser-assisted cataract surgery with the IOL implantation. In all cases studied, a comprehensive preoperative examination was performed. Age varied from 58 to 83 years (average 65.3 ± 11.2 years). Theimmature cataracts predominance was show in the study group (87 eyes-88.9%). Before the surgery all patients were prescribed antibiotics and NSAIDs. Operations were performed by one surgeon. The femtosecond laser step of the operation was carried out using a LensX femtosecond laser (Alcon, USA) with the laser exposure duration of an average of 50 seconds.

Results:

The lens fragmentation is performed using the femtosecond laser energy in four parts in the middle third of the lens thickness, bounded by planes parallel to each other and perpendicular to the sagittal plane. In the study group pupil diameter before the surgery was 6.95 ± 1.23 mm, after - 6.75 ± 1.17 mm. Even the initially narrow pupil with a diameter of 5.0 mm -5.5 mm (8 eyes-8.1%), has not narrowed and remained stable throughout the operation. In all cases, the proposed method allowed to keep the stable mydriasis without the use of additional viscoelastic, phenylephrine and pupil-expansion devices.

Conclusions:

Translocation of the laser energy application point to the maximum distance from the iris in combination with the preoperative use of NSAIDs makes it possible to maintain stable intraoperative mydriasis without the intraoperative use of medical and mechanical means. The proposed method of FLACS allows the surgeon to perform surgery safely and stress-free.
