Comparison of clinical outcomes between customized and non-customized corneal cross-linking

Abstract Details

Purpose:

To compare the visual and refractive outcomes between customized corneal cross-linking (CXL) and conventional CXL.

Setting:

Nagoya Eye Clinic, Nagoya, Japan Sato Yuya Eye Clinic, Sendai, Japan

Methods:

We performed a retrospective prescreening of patients who underwent CXL to prevent the progression of keratoconus. Among them, consecutive cases that could be followed up for more than 6 months were included. Epi-on custom CXL group (group C, 24 eyes of 18 patients, average K value 47.04 ± 3.39D) and conventional epi-on CXL group (N group, 31 eyes of 25 patients, average K value 47.9±3.98D). The visual function, refraction, and higher order corneal aberrations were examined in each group before and after surgery. In both groups, CXL was performed in a high oxygen environment.

Results:

In the group C, improvement in uncorrected visual acuity (p=0.0021), Kmax (p=0.0059), coma aberration (p=0.026), and total higher-order aberrations (p <0.0001) at 6 months after surgery was found after surgery. On the other hand, in the N group, there was no significant difference in all parameters including visual function, higher-order aberration, and Kmax before and after the surgery. No significant difference was observed in the corneal endothelial cell density before and after the surgery in both groups. In the N group, at 6 months after surgery, one case in which Kmax increased by 1D or greater was observed. One patient in group C had delayed postoperative epithelial wound healing (requiring longer than one week for epithelialization).

Conclusions:

The Epi-on CXL using the custom method can improve the corneal shape and the visual function from the early postoperative period compared to the non-custom method.
