Asymmetrical base intracorneal ring segment implantation for the treatment of keratoconus

Abstract Details

Purpose:

The main objective of this presentation is to describe the development process of the asymmetric base segment, to describe the surgical technique and assess the safety and efficacy.

Setting:

All surgeries were performed in the cornea department of Instituto Zaldivar de Mendoza, Argentina.

Methods:

Inclusion criteria were consecutives eyes of keratoconic patients identify as Duck; phenotype describe as paracentral ectasia, disabling visual acuity in absence of corneal scars or striae. All patients were evaluated with complete ophthalmological examination including age, gender, selected eye, visual acuity (UDVA), corrected distance visual acuity (CDVA), slit lamp examination, endothelial cells evaluation and funduscopy. Corneal topography for pattern identification and corneal OCT were performed previous and postoperative.

Results:

A retrospective analysis was performed, 30 eyes were implanted with the asymmetrical based ICRs after topographic patter identification. After one-year of followup, the mean UDVA improved by 0.62 logMAR ± 0.19 (SD) The mean CDVA improved by 0.18 ± 0.1 . The spherical equivalent decreased by mean of 3.1 ± 2.87 D. Keratometry (K) KMax decreased by a mean 2.24 ± 1.61 D. No complications were reported in the study group.

Conclusions:

The identification of the corneal keratoconic pattern is mandatory to achieve the correct selection of ICRS as well as the number and position of ICR for each ectatic cornea. Implantation of the asymmetrical base ICRS is an option allowing the customized treatment of keratoconus which is safe and reproducible.
