Corneal ulcers following exposure keratitis in intensive care unit

Abstract Details

Purpose:

to raise awareness about the importance of eye care during intensive care unit (ICU) stay by describing the most common ocular findings and their outcomes

Setting:

Adult ophthalmology department, 20 august hospital, Ibn Rochd university Hospital, Faculty of medicine, Hassan the 2nd university, Casablanca, Morocco

Methods:

We report a cross-sectional study of files of all patients that presented corneal ulcers during their ICU stay in our hospital between January 2018 and June 2019

Results:

we had 47 patients, the mean age of our patients was 51 years (SD: 10.4) with a male predominance (male / female sex ratio of 3/1). The average duration of hospitalization was 16 days (SD/ 4.2). Respiratory infection was found in 44.68% of which 10.63% needed a tracheotomy, sepsis was found in 19,4% and local or regional infection in 6.38%. The unilateral inferior corneal ulcer was found in 64%. An ulcer taking the entire cornea with superficial punctate keratitis of the contralateral eye in 15% of patients. Corneal ulcers were bilateral in 4% of cases. The corneal sampling allowed to isolate Pseudomonas aeruginosa in 30% of cases, Acinetobacter in 4% and sterile samples in the other cases (66%). All patients were initially placed under broad-spectrum antibiotics and adapted if need be. The evolution was marked by the disappearance of the ulcer in 75% of the cases, an ocular perforation in 12% and an endophthalmitis in 12%.

Conclusions:

Corneal ulcers on exposure keratitis in the intensive care unit are a frequent entity, often unknown, with formidable complications hence the importance of raising awareness about the importance of ophthalmic care provided during the stay in intensive care units and setting up an eye care guideline
