Evaluation of induced refractive astigmatism in patients who were implanted with Alcon Clareon IOL in one eye and Alcon Acrysof IOL in the other eye

Abstract Details

Purpose:

As cataract surgery is constantly evolving, a key area of innovation involves intraocular lens(IOL) design and biomaterial. Induction of against the rule(ATR) astigmatism after cataract surgery with a monofocal lens is thought to be also due to the posterior corneal surface. Alcon Clareon CNA0T0 is a monofocal IOL, made of hydrophobic acrylic material. It has exactly the same optic design and asphericity as the Acrysof IOL with only differences the material and location of aspheric surface (anterior vs posterior). The purpose of our study was to investigate whether IOL aspheric surface and/or material could play a role in inducing astigmatism.

Setting:

LMVision Clinic- Laser and Microsurgery Day Clinic - Thessaloniki, Greece

Methods:

Twenty two eyes of 11 randomly selected patients with preoperative regular corneal astigmatism equal or less than 1 diopter(dpt) were included in this pilot study. In every patient Alcon Clareon(CNA0T0) IOL was randomly implanted in one eye and Alcon Acrysof(SN60WF) IOL in the other. All operations were performed by the same surgeon. Preoperative and postoperative corneal and refractive astigmatism were measured with a Topcon combined keratorefractometer and topographer calibrated and thoroughly validated. Capsulorhexis’ circularity, roundness and centricity were estimated.

Results:

Surgically induced corneal cylinder was around 0.12dpt or less in all eyes. Vector analysis of induced refractive cylinder revealed 0.31dpt mean ATR induction with the AcrySof IOL (9 of 11 eyes angle between 75-105). Mean cylinder induction with the Clareon IOL was 0.04dpt. Among them 2 had no induced cylinder, 3 with-the-rule (mean 0.28dpt, angle 172-184) and 6 ATR cylinder induction (mean 0.27 dpt, angle 85-95). Capsulorhexis circularity, roundness and centricity were 0.99(0.97-1.00), 0.97(0.94-1.00) and 140μm (90-190) respectively.

Conclusions:

This pilot study indicates that Alcon Clareon CNA0T0 may induce less against-the-rule astigmatism compared to Alcon Acrysof (SN60WF). The novel design and biomaterial of this new IOL could probably induce less aberrations. Lens material and/or aspheric surface could play a role in the induction of ATR cylinder among with the posterior corneal surface. A larger number of patients is needed for definite conclusions.
