IOL calculation in eyes with multiple previous keratorefractive procedures

Abstract Details

Purpose:

We have become increasingly more accurate at calculating appropriate IOL power for eyes which have undergone a keratorefractive procedure such as myopic or hyperopic LASIK, PRK and even radial keratotomies. However, very limited recommendations or research exist regarding eyes which have undergone multiple keratorefractive procedures. This case series and discussion will outline how we have successfully approached this scenario and give guidance for such future cases.

Setting:

Private practice in Auckland, New Zealand

Methods:

Two eyes which had previously undergone radial keratotomies followed by LASIK and two eyes which had previous LASIK with multiple myopic and hyperopic enhancements underwent cataract surgery. Three of these eyes received a small aperture IOL and one a monofocal toric IOL. Their previous multiple keratorefractive procedures were taken into account and multiple IOL calculation techniques were considered. These will be discussed and potential refractive outcomes analysed.

Results:

Using our recommended methods, all eyes achieved unaided distance visual acuity better than 6/6 and had good quality of vision. We have produced a recommendation for dealing with eyes that have had multiple keratorefractive procedures.

Conclusions:

Eyes that have had multiple keratorefractive procedures need to be considered separately to eyes that have just had a single procedure. Refractive errors could occur if only the most recent procedure is taken into account. Good refractive outcomes from cataract surgery are possible and achievable for these types of eyes.
