Combined ultrasound with low IOP for hard nuclei removal

Abstract Details

Purpose:

The purpose of this study was to assess the main ultrasound and hydrodynamics parameters after hard nucleus removal using combined (longitudinal + torsional) ultrasound setting with low IOP

Setting:

S. Fyodorov Eye Microsurgery Federal State Institution. Saint-Petersburg. Russia.

Methods:

53 patients with hard nuclei (NC 6+, LOCS III) underwent FLACS procedures. In order to objectifying the comparison we used «Femtocomparison» technique. Femtosecond laser divided the nucleus into 4 identical quadrants, two of them were removed with combination of torsional and longitudinal US and the other two quadrants with Intelligent Phaco technology. In all cases low IOP (34 mm.Hg.) was used during the surgery (Centurion vision system).

Results:

Cumulative dissipated energy and aspiration time were lower when combination ultrasound was used. (p<.05). Insufficiency of torsional energy and low longitudinal energy in IP technology during hard nuclei aspiration leads to increase aspiration time and consequently increase dissipated energy.

Conclusions:

The combination of torsional and longitudinal US with close to physiological IOP, is more effective and safe than IP technology for hard nuclei removal.
