Evaluation of axis alignment of toric intraocular lenses using surgical guidance system: comparison manual axis registration

Abstract Details

Purpose:

To evaluate axis alignments during the implantation of toric intraocular lenses (IOLs) using surgical guiding system, comparing with a manual axis registration (AR) technique utilizing the cornea-conjunctiva marking and topography examination just before surgery.

Setting:

Miyata Eye Hospital, Miyazaki, Japan

Methods:

The retrospective study reviewed clinical records of 168 eyes of 143 patients who underwent cataract surgery with implantation of toric IOLs (SN6AT3-6, Alcon) and were intended for emmetropia. A surgical guiding system Version (Alcon) or manual AR technique was used for the axis alignment after IOL implantation. Toric calculators used were utilizing Holladay/Barrett method and conventional calculator based on fixed rate, respectively. The manifest cylinder and uncorrected distance visual acuity (UDVA) 1 month postoperatively were compared.

Results:

There were 9, 42, 68, 49 eyes with toric models of T3 to T6, respectively, and the axis alignments of Version and AR were used for 49 and 119 eyes. No difference between the groups. The mean postoperative cylinders with the use of Version was 0.50-0.86 D, and there was no difference from the use of AR technique (0.33-0.77 D) for all the toric models (P>0.31). In the UDVA, there was no difference in the use of the 2 types of axis alignments (P>0.13).

Conclusions:

The surgical guiding system that does not require complicated procedure was effective for toric IOL axis alignment as the postoperative outcomes were comparable with like AR technique.
