Preoperative ocular characteristics predicting the development of intraoperative floppy iris syndrome

Abstract Details

Purpose:

The goal of this study is to characterize the association between preoperative measurement values of ocular landmarks and the development of intraoperative floppy iris syndrome (IFIS).

Setting:

Hospital

Methods:

In this retrospective study we reviewed medical records of patients who underwent cataract surgery by phacoemulsification in Yitzhak Shamir medical center, between July and September 2019. Patients younger than 50 years, with preexisting ocular conditions affecting pupillary size or anterior chamber depth (ACD), and who underwent combined procedures were excluded. We collected demographic data, medical history including known risk factors for IFIS, preoperative cataract density and pupil measurements and biometric values. IFIS appearance and additional intraoperative and postoperative complications were recorded.

Results:

400 eyes of 394 patients were allegeable. Mean age was 72.5±8.6 years, 59% were female. IFIS occurred in 18 eyes (4.5%). Patients with IFIS were significantly older (78.1±6.8 vs. 72.2±8.6 years, p=0.005). Pupil diameter was significantly smaller in IFIS (5.73±1.2 vs. 6.97±1.0 mm, p<0.001). Lens thickness (LT) was higher in IFIS (4.94±0.54 vs. 4.48±0.53 mm, p=0.001). On univariate analysis ACD was significantly shallower in IFIS, but after adjusting for age, pupil diameter and LT the difference became nonsignificant. Additional measurements, including axial length and white-to-white distance were not found related to IFIS.

Conclusions:

Lens thickness and pupil diameter after pharmacological mydriasis are two significant risk factors for IFIS development on preoperative ocular examination, regardless of alpha-antagonist treatment status.
