Abstract Details

Purpose:

To assess the clinical characteristics and treatment outcomes of childhood glaucoma following cataract surgery.

Setting:

A multicenter, retrospective chart review study.

Methods:

This retrospective multicenter study included all patients diagnosed with glaucoma following cataract surgery according to the Childhood Glaucoma Research Network (CGRN) Classification criteria and had follow-up visits for at least one year, between January 2009 and December 2018, from Rajavithi Hospital and Queen Sirikit National Institute of Child Health, Thailand.

Results:

A total of 68 eyes from 43 patients were included in this study. Glaucoma occurred more frequently in aphakic (52.2%) than pseudophakic eyes after cataract surgery. A male predominance of 66% was found and the mean age of onset was 95.40+/-55.24 months. The mean age of cataract surgery and mean onset of glaucoma after cataract surgery were 17.25+/-28.09 and 86.29 +/-52.91 months. The most common type of cataract surgery was pars plana lensectomy (65.7%). About 40% of cases needed glaucoma surgery in which trabeculectomy was the most common. The complete & partial success rate of treatment were 12% and 55%.

Conclusions:

Childhood glaucoma following cataract surgery is usually severe and challenging in terms of treatment. Pars plana lensectomy is the major type of cataract surgery. Surgical and medical treatments are fairly effective. Glaucoma screening after cataract surgery is essential for early detection, especially in aphakic patients. Lifelong follow-up is required.
