Abstract Details

Purpose:

To report a case of Capsule distension in an eye with previous LASIK surgery and to discuss management options

Setting:

E;ite Eye Clinic and Maidstone Hospital, UK

Report of Case:

A 63 year old lady was referred for consideration of cataract surgery in both her eyes, with vision of 6/9 OD and 6/19 OS. Both eyes had myopic LASIK 30 years ago. The patient requested to aim for distance vision as she was comfortable wearing reading glasses. IOL Master and ASCRS calculator were used to calculate IOL power required for both eyes. The left eye was operated first followed by the right eye a week later. The patient’s UCVA one week after the second eye was 6/7.5 OD and 6/30 OS, with refraction of -0.5/-0.75x170 OD and -1.00/-0375x170 OS. The patient suffered blurry eye sight in her left eye but was pleased with right VA. Options discussed included IOL exchange for post LASIK refractive surprise. Sheimpflug imaging and AS-OCT scanning showed capsule distension in left eye. YAG capsulotomy was avoided to keep the option of IOL exchange if required later. Rentrolenticular wash out was performed few days later, and UCVA improved to 6/7.5 on day 1 post wash out.

Conclusion/Take Home Message:

Capsule distension syndrome should be considered as a cause of refracive surprise even in cataract surgery performed in eyes previously treated with LASIK. Retrolenticular washout should be the first treatment option over YAG capsulotomy to preserve the capsular bag in case IOL exchange is required
