Evaluation of dry eye disease in newly diagnosed anxiety and depression patients using anterior segment optical coherence tomography

Abstract Details

Purpose:

Dry eye disease (DED) is common in psychiatric patients , especially with longer duration of disease and use of medication. In addition, newly diagnosed psychiatric conditions may have a relation with DED according to findings about serotonin changes in human tear. We aimed to evaluate DED in newly diagnosed anxiety and depression patients using anterior segment optical coherence tomography (AS-OCT).

Setting:

This prospective study was designed and performed at ophthalmology department of Başkent University, School of Medicine, Konya Research Hospital.

Methods:

Thirty-four newly diagnosed patients with anxiety, 40 newly diagnosed depressive disorder patients from the pschiatry clinic and 38 age and sex matched controls were included in the study. BECK anxiety inventory was completed by anxiety patients and BECK depression inventory was completed by depression patients, to assess the severities of diseases. The Ocular Surface Disease Index (OSDI) was completed by all participants. Tear break up time (TBUT), Schirmer’s test and ocular surface vital dye staining (OSS) were performed. Tear meniscus parameters were evaluated as Tear Meniscus Depth (TMD), Tear Meniscus Height (TMH) and Tear Meniscus Area (TMA) using AS-OCT.

Results:

There were no significant difference in age and gender. (p=0.765, p=0.658) Anxiety patients were group 1, depression patients were group 2 and controls were group 3. Schirmer’s tests (mm) were 7.24±6.02, 6.58±4.9 and 18.79±4.9 and TBUT (sec) scores were 5.62±3.1, 5.6±3.5 and 13.37±1.7, respectively. OSDI scores were 28.01±19, 30.43±18.49, 14.38, and OSS scores were 1.9±0.7, 2.1±0.6 and 0.7±0.4, respectively. TMD, TMH and TMA values were significantly lower in anxiety and depression patients than controls.

Conclusions:

In this study, we showed that the newly diagnosed anxiety and depression patients may have association with DED. However, in patients with depression, association between DED and severity of disease is more prominent that anxiety patients. In addition, the non-invasive techniques as AS-OCT can be helpful to assess DED in these patients. According to our results, we can suggest the ophthalmologic examination for psychiatry patients before the beginning of medication.
