Abstract Details

Purpose:

Excimer laser surgery, implantation of implantable contact lenses and refractive lens exchange have all proven to be an option to correct presbyopia. But when and why to use which option is still an unsolved challenge.

Setting:

All surgeries were performed by one surgeon at the Breyer-Kaymak-Klabe Eye Surgery and Premium Eyes in Duesseldorf, Germany, member of the International Vision Correction Research Center (IVCRC.net).

Methods:

As accommodative optics are not yet established, we face two challenges: What is the best optical solution and how do we classify associated complications. We searched literature to find best practice solutions optical and complication wise.

Results:

Excimer laser surgery has the least potential risk of complications and least disturbing optical side effects followed by the implantation of contact lenses and IOL.

Conclusions:

First line therapy in the correction of presbyopia should be excimer laser surgery in clear crystalline lenses. The implantation of presbyopia correcting implantable contact lenses is an interesting option for patients looking for a removeable procedure. Refractive lens exchange should be reserved for cataractogenous lenses and medical indications.
