Abstract Details
Purpose:
To present a case with traumatic posterior scleral perforation due to an intraocular foreign body, managed with pars plana vitrectomy, intraocular foreign body removal and intraoperative closure of the posterior scleral defect with a blood clot.
Setting:
Necmettin Erbakan University, Meram School of Medicine, Ophthalmology Department
Methods:
A 34-year old man presented with corneal perforation and dense vitreus hemorrhage after a gunshot injury. The visual acuity was hand motions. Orbital computer tomography revealed a metallic foreign body embedded in sclera at the posterior pole of the left eye. A three port surgery including lensectomy, foreign body and vitreus hemorrhage removal was performed. After removal of the foreign body, a 3 mm defect in the inferior arcuate was detected. Blood drawn from the antecubital vein of the patient was placed on the defect and argon endolaser photocoagulation was performed to seal the tissue. Heavy silicone oil was given.
Results:
Silicone oil was removed three months after the first procedure. Patient was followed for one year. Final best corrected visual acuity was 0,5 (Snellen Chart). Optic disc and macula were normal in appearance. A chorioretinal scar was observed on the inferior arcuate fibers inferotemporal to the macula.
Conclusions:
Blood clot plug may be considered in closure of small posterior pole scleral defects in globes with double perforation due to foreign body trauma.
