Abstract Details

Purpose:

The authors discuss the use of micropulse transscleral laser therapy (MP-TLT) in patients with glaucoma, and the assessment of efficacy and safety in patients with secondary glaucoma after previous cataract surgery and pars plana vitrectomy

Setting:

Methods:

From September 2019 to September 2020, Cyclo G6 (IRIDEX, Mountain View, California, USA) micropulse laser cyclophotocoagulation procedures were performed in 20 patients after previous cataract surgery and pars plana vitrectomy. 15 women and 5 men aged 36-95 years (mean age 65.38) with secondary glaucoma, for whom local treatment was insufficient, were qualified for the procedure. A laser with a wavelength of 810 nm with interrupted pulses of 2000mW power was used. Before the operation and during the observation period, the following were analyzed: distance visual acuity, IOP, the number of active substances in the used drugs and postoperative complications

Results:

During the observation period, the number of active substances in the drops was at a similar level (2.14) as before the surgery (2.25) . However, throughout the observation period 1, 3, 6 Mo, a significant reduction in IOP was observed compared to IOP before surgery, reaching the value within the normal range (p<0.05). At the same time, visual acuity remained at a similar level (0.18 and 0.24 respsctively). No side effects were observed, only a slight temporary inflammation of the anterior segment of the eye related to the procedure itself

Conclusions:

Micropulse laser transscleral therapy (MP-LTL) with the use of Cyclo G6 is an effective and safe method of lowering intraocular pressure, also in the treatment of secondary glaucoma after posterior vitrectomy. Moreover, it may postpone the anti-glaucoma surgery, including its performance in the future. As the results show, this procedure is safe and does not deteriorate vision in patients
