Evaluation of macular inner retinal layers in patients with Behçet’s disease without ocular involvement

Purpose:
The aim of this study was to evaluate the changes in macular inner retinal layers, total macular volume (TMV) and central macular thickness (CMT) in patients with Beh�et�s disease (BD) without ocular involvement.
Setting:
This cross-sectional study was conducted at the Department of Ophthalmology, Necmettin Erbakan University Meram Medical Faculty Hospital, Konya, Turkey.
Methods:
Thirty-five patients with BD and 30 healthy control subjects were included in the study. Patients with signs of recent or previous ocular involvement of BD were excluded. All subjects underwent spectral-domain optical coherence tomography (Spectralis OCT, Heidelberg, Germany) imaging, and retinal layer segmentation was performed automatically. In addition to TMV and CMT, the average thicknesses of nerve fiber layer (NFL), ganglion cell layer (GCL), inner plexiform layer (IPL) and inner nuclear layer (INL) were measured at the center (1 mm) and four quadrants (superior, inferior, nasal, temporal) of the parafoveal region (1-3 mm). Only right eyes were included in analyses.
Results:
The mean age was 40.7 � 8.6 years in patients with BD and 41.3 � 8.6 years in the control group. There were no significant differences between the two groups in terms of age (P=0.807) and gender (P=0.321). Patients with BD showed significant reductions in TMV (P=0.022), CMT (P=0.003), and thicknesses of NFL-central (P=0.007), NFL-superior (P=0.009), NFL-nasal (P=0.047), GCL-central (P=0.004), GCL-superior (P=0.039), GCL-inferior (P=0.041), GCL-nasal (P=0.002), IPL-central (P=0.019), IPL-nasal (P=0.044), and INL-nasal (P=0.036) regions. The duration of disease inversely correlated with IPL thickness at the central (r=-0.401; P=0.017), inferior (r=-0.399; P=0.018), and nasal regions (r=-0.365; P=0.031).
Conclusions:
Total macular volume, CMT and thicknesses of macular inner retinal layers were reduced in patients with BD even in the absence of any symptoms and signs of ocular involvement. Additionally, the reduction in IPL thickness was associated with disease duration. These findings suggest that subclinical retinal nerve degeneration occurs during the course of BD.
