Corneal epithelial pachymetry parameters in keratoconic eyes

Abstract Details

Purpose:

To evaluate corneal epithelial pachymetry map details in keratoconic eyes according to the clinical stages of the disease.

Setting:

Oftalmika Eye Hospital, Department of Ophthalmology, Faculty of Medicine, Collegium Medicum in Bydgoszcz, Nicolaus Copernicus University in Torun, Poland

Methods:

Retrospective analysis of 45 eyes with keratoconus and 55 normal eyes. All keratoconin eyes were divided into groups based on the Amsler � Krumeich classification: stage 1 (34 eyes), stage 2 (11 eyes). Mean corneal power in the steepest meridian was 45.03D (�3.16D) in stage 1, 50.59D (�2.95D) in stage 2 and 44.62D (�1.73D) in control group. Amount of astigmatism, mean corneal power were measured with Sirius (Schwind, Germany) corneal tomography and topography. Corneal epithelial maps were obtained using an anterior segment OCT (Avanti, Optovue, USA). Differences between all three groups in corneal epithelial thickness profile were analysed.

Results:

Average central epithelial thickness was 54.25um (�3.01) in control group, 50.94um (�8.06) in stage 1 (p<0.05) and 50.25um (�4.78) in stage 2 (p>0.05). Mean minimal epithelial thickness was 49.27um (�3.11) in control group, 41.55um (�7.52) in stage 1 group (p<0.05) and 39.92um (�5.85) in stage 2 (p>0.05). Significant differences between all three groups were in a central and minimal corneal thickness and apex curvature. There were no significant differences in mean paracentral (2-5mm) epithelial thickness in superior and inferior quadrants.

Conclusions:

There was a significant difference between normal and keratoconic eyes in central and minimal epithelial thickness. Although in stage 2 the mean central and minimal epithelial thickness were less than in stage 1, the difference was not statistically significant. Analysis of corneal epithelial profile can be useful in detection of early keratoconus stages.
