Role of adaptive optics aberrometer in keratoconus laser planning

Abstract Details

Purpose:

To assess the role of Adaptive Optics Aberrometer to customize selective aberration correction in a Keratoconus laser planning

Setting:

Narayana Nethralaya, Bangalore

Methods:

32 eyes with mild to moderate keratoconus were recruited & visual adaptive optics was performed preoperatively. Visual adaptive optics stimulator(VAO)モVoptica (Murcia, Spain) was used to study visual improvement in keratoconic eyes based on which we decided whether a particular aberration was to be treated or not. All aberrations were corrected in 16 eyes(group 1)&spherical aberrations were not corrected in 16 eyes (group 2) . Customization of aberrations was done on SCHWIND-AMARIS 1050 ORK-CAM software basis our findings on VAO after which cornea was ablated. Post-operative visual & aberrometric outcomes in group 1 & 2 were compared at 3&6 months

Results:

Outcomes were assessed on number of loss of lines, gain of lines & improvement in quality of vision.There was a gain of 1 line in3 eyes, loss of 2 lines in 6 eyes in group 1 & gain of1 line in 8 eyes,loss of 1 line in 4 eyes in group 2.In group1 there was a significant change in coma, trefoil & spherical aberrations (p less than 0.001). In group 2 there was significant change in coma & trefoil (p less than 0.001) but no significant change in spherical aberration.Quality of vision improved more in group 2 than group 1

Conclusions:

Visual Adaptive Optics helped us to evaluate aberrations which suited patientsメ eyes the best following which customization of laser treatment was done accordingly on SCHWIND-AMARIS 1050 ORK-CAM software. This helped in improving the quantity and the quality of vision post-operatively as it is suggested in some studies that it is not necessary to correct spherical aberration completely. Positive spherical aberration was correlated with better visual outcomes.
