Treatment of post-inflammatory scarring of the cornea: after herpetic stromal keratitis, occurred after cataract surgery, topical application of matrix regeneration therapy (RGTA)

Abstract Details
Purpose:
Ophthalmic herpes simplex viral keratitis is responsible for a wide range of ocular manifestations from superficial epithelial disease to stromal keratitis and endotheliitis. In Europe, herpes simplex virus type I (HSV) is a common cause of keratitis. The disease may be well treated if the ophthalmologist is aware of the various types of clinical expressions of this typical unilateral chameleon, and the treatment is adjusted accordingly. A significant part of inflammations of the cornea during treatment proceeds in a chronic form. In some cases, the cornea develops keratopathy or post inflammatory scarring.
Setting:
Outpatient Clinic in University Clinical Hospital, Department of Ophthalmology, Medical University of Silesia, Katowice, Poland.
Methods:
A health 75-year old male patient two months earlier underwent phacoemulsification cataract surgery with lens implantation of the left eye at different hospital. After the procedure eye drops were applied: an antibiotic (5x a day for 14 days), a steroid (4x a day for 21 days) and non-steroidal anti-inflammatory drug (2x a day for 21 days). From the interview 14 days after surgery, the patient developed herpes labialis and reported redness and severe pain in the left eye.
Results:
In view of the the improvement of the cornea and BCVA we decided to use next dose of Cacicol drops. Throughout the treatment the patient has applied 3 treatment cycles of Cacicol (each dose 1x a week for 5 weeks) and between each cycle we made four weeks break. After a series of three cycles of Cacicol we found vision improvement of the left eye (BCVA to 0,6) and we also found an improvement in corneal transparenty, topography and pachymetry map parameters.
Conclusions:
1. HSV keratitis can occur after cataract surgery in patients with herpetic history. 2. The long-lasting HSV keratitis can occur scarring, opacity and thinning of the cornea. 3. Cacicol belongs to the regenerating agent family (or RGTA) can accelerate the recovery of the cornea, improve the visual acuity and transparency of the cornea and it can be used in post-inflammatory scarring of the cornea after herpetic stromal keratitis. 4. Cacicol is safe and easy to use.
