Corneal confocal microscopic analysis of endothelial cell layer in patients with Behçet’s disease

Abstract Details

Purpose:

To evaluate corneal endothelial cell alterations in patients with Beh�et�s disease (BD) with and without a history of uveitis using in vivo corneal confocal microscopy (CCM).

Setting:

This single-center study was conducted at the Department of Ophthalmology, Necmettin Erbakan University Meram Medical Faculty Hospital, Konya, Turkey.

Methods:

Forty-five consecutive patients with BD and 40 healthy control subjects were enrolled in this cross-sectional study. Among patients with BD, 16 (35.5%) had a previous history of uveitis involving the anterior segment of the eye. Patients with active uveitis or any other ocular disease were excluded from the study. The laser scanning CCM (Rostock Cornea Module/Heidelberg Retina Tomograph lll; Heidelberg, Germany) was used to obtain endothelial images from the central cornea and the mean endothelial cell densities were evaluated using the software provided in the CCM device.

Results:

There were no significant differences between patients with BD and healthy control subjects in terms of age (P = 0.127) and gender (P = 0.703). The mean endothelial cell density was 2583 � 240 cells/mm2 in patients with BD and 2698 � 207 cells/mm2 in the control group (P = 0.022). Subgroup analysis revealed that BD patients with a history of uveitis had lower endothelial cell density compared with healthy subjects (P = 0.038), while no significant difference was observed between BD patients without uveitis and the healthy control group (P = 0.267).

Conclusions:

This study demonstrates significant reduction in corneal endothelial cell density in patients with BD, particularly in the subgroup of subjects with a history of uveitis. There was no difference in endothelial density between patients without uveitis and healthy subjects, nevertheless, further longitudinal studies are needed to confirm these findings.
