Outcomes and prognosis of corneal transplantation performed on geriatric patients

Abstract Details

Purpose:

This study aimed to evaluate indications, outcomes and prognosis in geriatric patients who underwent corneal transplantation.

Setting:

Akdeniz University Ophthalmology Deparment

Methods:

Data of the patients older than 65 years who underwent corneal transplantation between January 2000 and January 2019 were collected. They were followed up for at least 1 year. Two hundred four eyes of 199 patients were retrospectively analyzed. Age, gender, time between the onset of complaints and transplantation, duration of follow-up, other intraoperative-postoperative surgical procedures, complications, intraocular pressure (IOP), best corrected visual acuity (BCVA) were recorded. The data were statistically analyzed using paired-t test. P<0.05 was considered to be significant.

Results:

The study included 104(53.4%) males and 95(46.6%) females patients.Mean age was 71.9�5.8(65-95) years.Types of keratoplasty procedures were 189(%92.6) penetrating keratoplasty,9(%4.4) Descemet's membrane endothelial keratoplasty and 6(%2.9) deep anterior lamellar keratoplasty.Preoperative indications were bullous keratopathy(BK) in 102(50%),corneal scar in 49(24%),corneal dystrophy in 17(8.3%),spontaneous corneal perforation in 17(8.3%),corneal abscess in 9(4.4%),graft failure in 6(2.9%),others in4(%2) patients.There were 90(88.2%) pseudophakic and 12(17.8%) aphakic BK patients.Thirty-three BK patients (36.7%) had anterior chamber-IOL and 15(16.6%) had sclerally fixated-IOL.Graft rejection was observed in 58(28.5%) eyes.23(11.3%)eyes which are unresponsive to medical treatment underwent re-keratoplasty.Postoperative glaucoma diagnosed in 50(%24.5)eyes and of them 11(5.4%) necessitated trabeculectomy.Preoperative-postoperative BCVA was 2.1(range,1.5-3.1),1.0(range,0.6-2.1)(logMAR),respectively(p<0.001).

Conclusions:

The most common indication for corneal transplantation was BK in geriatric patients. In the elderly, although the visual outcomes are satisfactory, there is an increased risk of postoperative glaucoma which together with high incidence of systemic diseases can affect the graft survey and visual outcomes.
