Abstract Details

Purpose:

To report a challenging diagnosis and surgical management of a subluxated lens.

Setting:

Case report with anterior segment optical coherence tomography (AS-OCT) findings and surgical case.

Methods:

Case report of a healthy 43-year-old female, with a sudden decrease in the left eye vision. The patient presented increased myopia over the past 4 months (from a stable refraction of -02.0D to -08.0D) and shallow anterior chamber, which then resulted in acute angle closure and pigment dispersion. Patient had no history of trauma or systemic disease.

Results:

The patient was referred to a glaucoma specialist, due to glaucoma/pigmentary dispersion syndrome suspicion, 3 months after the initial observation. At this point, an anterior and temporal lens subluxation was evident and the patient was submitted to phacoemulsification surgery using iris hooks to suspend the capsular bag. Capsular tension ring and intraocular lens were implanted in the capsular bag and fixated to the sclera with 10�0 prolene. No intraoperative complications were reported. One week postoperatively, the patient presented with best corrected visual acuity of 20/20.

Conclusions:

Phacoemulsification of a subluxated lens is a surgical challenge. In our case, the diagnosis remained elusive in the initial stage of the subluxation, presenting only as an increase in myopia and shallow anterior chamber.
